Referral process

A clear, five-step pathway from first contact to ongoing support.

    Referral Received

    We receive the referral and review the information provided to understand the individual's needs and circumstances.

  2. 2

    Initial Contact

    We make contact with the referrer and/or individual to discuss the situation, answer any questions, and agree the next steps.

  3. 3

    Home Visit & Assessment

    We visit the property to assess the home environment, understand the individual's goals, identify risks, and develop a tailored support plan.

  4. 4

    Practical Intervention

    We deliver hands-on support through decluttering, clearance, organisation and cleaning, working alongside the individual at their pace using a person-centred approach.

  5. 5

    Ongoing Support

    Where required, we provide follow-up visits and work collaboratively with professionals to help maintain progress and support long-term outcomes.
